Once you complete your return, the Tax Commission suggests that you consult the following check list: Did you attach the pre-addressed r" label to your return? If necessary, did you write your corrections right on the pre-addressed label? If you have no label, did you write your name, address and Social Security number on the return form? Did you double-check your Social Security number (and your spouse's, if applicable) to ensure that it is correct and legible? If you are filing a TC-40NR (nonresident or part-year resident return), did you attach a completed copy of your federal form 1040--including all forms and schedules and copies of returns filed with other states? Did you attach all state copies of W-2 or other forms showing Utah tax withheld? Did you sign and date the return on the back page? Remember, if it's a joint return, your spouse must sign too. Did you provide your daytime telephone number? Did you enclose payment of any additional tax due? Make your check payable to: Utah State Tax Commission. Please write your Social Security number beneath your signature on the check. Did you attach enough postage? Returns of six pages or more may require additional postage. If you have a refund due, did you completely darken the box on the front of the pre-addressed envelope? Did you attach all the supporting documentation to your return? Is your arithmetic correct? Are all of your entries on the proper lines? Is your handwriting readable? Did you use the right tax table or schedule to determine your tax? Did you make a copy of your return for your records?
