Mayor A. R. Hcywood, chairman of the city exemption board, today issued a special warning to all registrants to see that their local boards have their correct present address. Many cases have been found, he said, where men have changed their address since registering reg-istering several months ago and have not notified the board of the change. If there is aaiy fihange in the address and it Is not reported the questionnaire will bo sent to the address known by the board and the registrant will be held responsible for a proper account-l account-l in sr. H No Exemptions 1 Every man not in a training camp H now must fill out a questionnaire. He- H jection certificates, exemption certi- H ' ficates or discharge papers of any de- H scription will become invalid Decern- H ber 15, and all men not in the army H will be on the same footing. The local H board will take no notice of any pre- H vlous business of the registrant with H exemption boards, all registrants being H on the same now basis. H No information lias been received H yet concerning the date of the next H call or the number of men to be called. H Unofficially it has been rumored that the next call would be for 1,600,000 B men which would raise the quotas from H Weber county and Ogden city slight- H ly. If these two districts arc given J' credit for enlistments in the army and H navy since the first draft call, the quotas should be reduced to a com- Hi paratively low figure, as local men HI have been voluntarily joining the col- HL ors at a lively rate since the first call. H The government has issued no instruc- Hl tions regarding any phase of the work except the filling out of the qucstion- H naires. When these are filled out and all the men of military age in Weber county and Ogden city are classified H in one of the five divisions, it is ex- pected plans will be for calling the H men. into service. A number of instruction notes have H been provided the exemption boards H and one will be placed in each qucs- H tionnaire. They give the following H message: "Your questionnaire must be return - 1 ed within seven days after it is sent H )ut. If you need assistance in answer- HJ ing your questionnaire call at your lo- f cal board office or an attorney. No fee H hould be charged for service render- New Regulations H The order specifically says: "All e- H mptions and discharges made prior to H the date of these rales and regulations H and all certificates in .evidence there of are hereby revoked, and all such certificates heretofore issued shall have no further validity." This means that the local registration boards in each district shall proceed to classify all registered men, not already in the training camps, separating them into five groups. Asks 147 Questions To each registrant will be mailed a copy of the government's "questionnaire." "question-naire." This carries extended and specific spe-cific instructions as to how it shall be filled out and sworn to, and carries a total of 1-17 questions, separated into twelve "series." The blanks will be sent to 5 per cent of the registrants in each district on each day, beginning Dec. 10 in Chicago. The Individual has seven days from the date of mailing, excluding Sundays and legal holidays, to return the blanks properly filled out. : Ho is required to road the entire document carefully, making his answers an-swers to such of the 147 questions as he desires to answer, and then to return re-turn to page 1 of the form, where he makes a cross in the blank space before be-fore the separate classifications there enumerated. Can Waive All Claims If he prefers to waive all claims for exemption, there is a place for that. He then signs his name, and makes oath to the entire document before proper officers, who are stipulated in the questionnaire. Every registrant must answer all of the fifteen questions in Series 1. These are general and furnish a line on the capabilities and experience of the registrant. reg-istrant. They include questions as to full name, present age, occupation and residence, name and address of nearest near-est relative; employer's name and address; ad-dress; all occupations held during the last ten years and length of time in each occupation; number of hours per week occupied during the period since May IS, 1917, when the draft law became be-came effective; preferred occupation; previous military service; preferred branch of army service; and character and scope of education. Eighty-seven different occupations are then named and the registrant is required to draw one line under those at which he has worked and two lines under those In which he considers himself expert He is asked if he has been convicted of a crime, and if so, what, when and where; whether he Is ! confined in a prison or reformatory or ' is out on bail awaiting trial. Must Answer Two Questions ' Every registrant must answer the , two questions in series 2, covering . physical fitness. The first is: "State your weight and height stripped." The j second is, "Are you in sound health mentally and physically."' If the an- swer is "no" he is required to fur- nish details of his ailment of physical , deformity and the names and addresses address-es of physicians who have administered adminis-tered treatment during the last twelve months. If he is an inmate of an asy- 1 lum. Institution or hospital, he is re- -quired to give the facts. ' Every registrant must answer the ' first question by "yes" or "no," in 1 each of the series from 3 to 12 in- 1 elusive. Series 3 starts: "Are you ! a federal or state legislative, executive 1 or judicial officer9" If the answer is : "wo" ho answers no more questions I in the series. If "yes" he must stip-! ulate the exact designation of his office, when he entered office and when ho will cease to occupy it. Series 4 starts: "Are you a regular or a duly ordained minister of religion -and if so, what sect or organization?" If "no" he answers no more questions in that list. If "yes" ho gives the details de-tails concerning his religious labors and is asked If he follows any additional addi-tional occupation and if so, what? For Divinity Students Series 5 deals with divinity students: "Were you on May IS, 1917, a student stu-dent preparing for the ministry?" is the first question. If "yes" he must give the name and location of all divinity div-inity schools attended; whether any of them is wholly or partially a correspondence corre-spondence school, and if ho has ceased ceas-ed to be a divinity student since May 18. Answers in this series must be supported by affidavit. Series 6 concerns the military or naval-service. The first question is: "Are you in any branch of the mili-tary mili-tary or naval service of the United States?" If "yes" complete detail of rank, organization and duration of ser-vico ser-vico must follow. Aliens May Waive Exemption "Are you a citizen of the United States?" heads series 7. If the answer is "yes" no furtlier answers are required re-quired in the series. Question 2 is, "Do you claim exemption from military mili-tary service because you are not a citizen." citi-zen." If the. answer is "yes" the registrant reg-istrant must state where and when he was born; when and how he arrived in this country; whether by vessel or other means; whence he came; whether wheth-er he came with his parents; whether either of his parents has been natu-alized; natu-alized; whether he ever voted or registered reg-istered for voting in the United States and if so where whether he has taken out first naturalization papers, and concludes: "Are you willing to return to your native country and enter its military service?" Series S has to do with county or municipal officials, custom houso or postal employes or employes of the Unite'd States who have been designated desig-nated by the presidept as liable for exemption from military service. If the registrant comes within this classification he "must state the designation desig-nation of his office; whether elected or appointed; the exact place where he performs his duties; how long he has held office or employment; the character char-acter and duration of education, training train-ing and experience for his position; the nature of his work, and how many persons of same or similar grade of work are employed in his establish ment. Religious Convictions Considered. The subject of religious convictions is handled In Series 9. "Are you a member of a religious sect or organization organ-ization whose creed forbids you to participate in war in any form? If so state the name of the sect and the location lo-cation of its governing body or head," Is the question put to all registrants. If "yes" he must state tho date and place of the organization of his local church, the number of adherents of such sect in the United States, when such sect adopted opposition to war as part of its creed; when, here, and how he became a member of such sect, and the name and location of the particular local congregation to which he belongs. Dependency Important Factor. The highly important factor of do-pendency do-pendency is tho subject of Series 10. All registrants are required to answer tho first question: "Have you a wife or 'child or aged, infirm, or invalid parents, or grandparents, or brother under 16, or sister, under IS years of ago, or a helpless brother, or sister of whatever age, mainly dependent on your physical or mental labor for support?" sup-port?" If the registrant intends to claim exemption on- account of having dependents he answers the question with a "yes," and the following thirty questions must be supported fully by affidavits. If he does not intend to claim exemption for such reason he answers "no" to the first question and is not required to answer the other oth-er thirty. If he is claiming exemption and answers "yes" he states whether he is married, single, or divorced; when, where, and by whom he was married, if he is; name, address, age, and relationship of each person lnain-ly lnain-ly dependent upon him for support: which of such dependents lives with him and for how long; if any dependent depend-ent is a stepchild, adopted child, or foster child, when such relationship began; when he began contributing to the support of each dependent; the approximate total and th'e average monthly amount of his support to his wife and children during the last twelve months; how much he has contributed con-tributed to the support of each other dependent during the last twelve months; whether each of such dependents de-pendents is wholly dependent for support sup-port upon the registrant, if any dependent, de-pendent, other than wife or child, has a brother, father, mother, uncle, aunt, husband, wife, or child, and if so, age, address and occupation of each. Questioned as to Income. He is then asked what was his total income from all sources during the last twelve months; how much in cash and how much in other things of value"; how much was the fruit of his mental or physical labor; a full statement state-ment of all property owner or held in trust for him, whether income producing produc-ing or not, and the net income from same for the last year. Then he Is asked If he owns the house he lives in; If any of his dependents owns It; if he rents his house and if so tho monthly rent and name and address o.f his landlord; if he has paid any taxes in the last year, and if so, how much real estate and how much personal property, also income tax; and if there are any incumbrances of any kind upon any property ho owns, full details concerning same. He must state In money value all property owned or held In trust for any named dependent; depend-ent; tho earnings of each dependent for the last year; the amount of contributions con-tributions to each dependent's support made by persons other than the regis- 1 trant; the amount per month regis- ' trant considers necessary for the sup- port of all his dependents; whether ; his wife has been employed part or all ' of tho last twelve months, the nature ' of her employment and her earnings; 1 whether his wife ever has been em- : ployed and if so what was her calling; 1 whether his wife is trained or skilled 1 r ?y calImS! tQc condition of health 1 of his wife; full answer as to whether : his wife lives with her parents; and full answer as to how much he pays for his board or subsistence to any named dependent, if he lieves with 1 such dependent, other than his wife' or : children. j t Series 11 deals with industrial oc- '3 cupations. Every registrant must answer an-swer question 1, as follows: "Aro you i engaged in an Industrial enterprise ( (1) to the maintenance of the military establishment, or (2), to the effective operation of tho military forces, or (3), to the maintenance of the national nation-al interests during the emergency?" II "yes," and he claims exemption, he must answer twenty-two more questions ques-tions and must file two supporting affidavits. If he is an employe the affidavits must bo made by his Immediate Imme-diate superior and by the executive ol the enterprise. It he is In business for himself the affidavits must bo made by his co-partner, If he has one, and a near neighbor, or if ho has no co-partner by two near neighbors. The following questions cover the nature of the enterprise: its nanio and location; what It produces; how much time he gives to the enterprise; if not, what does he do; what trade name is applied to his job, such as "laborer," "skilled laborer," "foreman," "manager," "man-ager," etc.; the registrant's general duties; how long he has been engaged in such work, his education, training and experience for his work, and his reasons why he can not easily be replaced re-placed by another. Must State Earnings. An employe is required to answer further questions, covering the number num-ber of employes in the plant, how many persons employed in the same kind of work. A man in business for himself is required to state his earnings earn-ings per day, week or month; whether in business as an individual or copartner; co-partner; amount of capital he has in vested; how long he has been engaged in such enterprise; whether such enterprise en-terprise is a solvent, prosperous and going concern; his net earnings from such business for twelve months past; how many persons he employs, and whether he has any person related by blood, marriage or contract who can assist in the management of the enterprise, en-terprise, and why he can not take his place during the registrant's absence. Relates to Farmers. f Tho last series, 12, concerns agricultural agricul-tural occupations. Every registrant is asked: "Are you engaged in an agricultural agri-cultural enterprise?" If "not," he answers an-swers none of the following thirty-one questions. If "yes," and he claims exemption thereby, he must file two supporting affidavits. If an employe, one affidavit must be made by his employer, em-ployer, the other by a near neighbor. nn |
