Show LEGAL NOTICE REVIEW OF NEW INSTITUTIONAL HEALTH FACILITY Utah Dialysis Lab, Inc. is requesting approval of a capital expenditure of $48,000 for the purpose of 'establishing a new End Stage Renal Disease Facility. Uintah Basin Dialysis Center, a division of Utah Dialysis Lab, Inc., proposes to establish a free-standing limited care hemodialysis facility to be located on the premises of the Duchesne County Hospital. A public hearing will be conducted by the Utah Health Systems Agency if requested by any affected person no later than December 12, 1979, at the agencies listed below. Such public hearing, if requested, will be held on December 13, 1979, at Uintah Basin Area Vocational Center, Board Room, at 6:30 p.m., East Lagoon Street, Roosevelt, Utah, for the purpose of "'"""6 puuv. This project has limited impact on the health care system in terms of cost, availability and accessibility, ac-cessibility, and is eligible for expedited review which will exclude further public input unless substantive public controversy arises through a requested public hearing or as a result of negative planning plan-ning agency findings. The final decision on this proposal will likely be completed by December 20, 1979, unless the additional meetings are required which will delay the decision to as late as February 5, 1980. Comments on the proposal at the January 10 and 24 meetings will be allowed only by affected persons who file a Notice of Appearance and provide a written summary sum-mary of comments with supporting data. Notice of appearance must be received by January 2, 1980, at one of the offices listed below.
