Show shall be on separate bid schedules. Material procurement is to be completed in (30) calendar days or to comply with construction schedule and the project is to be completed prior to December 20, 1982. In conformity with, and by authority of Title 34, Chapter 30, Utah Code Annotated 1953, the general prevailing rates as determined by the Industrial Commission of Utah, shall be effective and apply to this project. The Brianhead Town Corporation in accordance ac-cordance with the provisions of Title VI of the Civil Rights Act of 1964, hereby notifies all bidders that it will affirmatively af-firmatively insure that the contract entered into pursuant to this ad-vetisement ad-vetisement will be awarded to the lowest responsible bidder, without discrimination of the grounds of race, color or national origin. Proposal forms, plans and specifications are on file in the office of Bulloch Bros. Engineering, Inc. Cedar City, Utah 84720 at 36 North 300 West and the Town offices at Brianhead, Utah 84719, where they may be reviewed by prospective bidders. Specifications, proposals forms, and plans may be obtained only at the above offices upon payment of a fee of $20.00, a portion of which will be refunded on return of same, ($10). Each bidder must have proof of valid Utah General Contractors license and bid bond from an approved ap-proved surety company; or in lieu thereof, cash, certified check, or cashiers's check for not less than 5 percent of the total amount of the bid, made payable to the Brianhead Town Corporation Cor-poration as evidence of good faith and a guarantee that if awarded the contract, the bidder will execute the contract bonds as required.
