It is officially approved by the passport office in Washington, Washing-ton, D.C., for travel purposes and is an officially accepted record re-cord of facts required by school entrance laws, and can be used for some sport programs, prog-rams, a health department news release says. The card is not intended to replace the certified copy of the birth certificate, but to be used in addition to it. It cannot be used for legal purposes requiring re-quiring proof of parentage or for social and welfare services. THE CARD, signed by Registrar Reg-istrar Enrico A. Leopardi and Deupty Registrar Doris H. Malm, will contain the local file number; the name, date and birthplace of the listed person; per-son; the date the birth certidi-cate certidi-cate was filed with the Health Dept.; the date the birth registration reg-istration card is issued and the Davis County Seal. Parents are encouraged to obtain a birth registration card in addition to a certified copy of the birth certificate to carry car-ry the card on their person and keep the birth certificate in a safe place at home with other important family records. ALL PARENTS with children chil-dren born in Davis County since Jan. 1, 1973, may make application for a wallet-sized card by contacting the Davis County Health Dept. The required re-quired fee is $6 each. |
