Mr. Powell explained that your most recent W-2 form furnished by your employer, or your last income tax return along with your Schedule C or Schedule F and the cancelled check showing payment of the self-employment social security tax, if you are self-employed, will suffice for proof of recent earnings. Mr. Powell added that a birth certificate is not necessary neces-sary for proof of age but that any old document giving your date of birth, an old family Bible, or an old insurance policy might possibly be all that is needed for proof of age. Mr. Powell cautioned that no one should delay filing a claim because he or she does not have one or more of the documents necessary to establish right to benefits.
