Show Utah School immunization law explained By Michael Harrison Within two weeks, schools will begin to open their doors for another year. The Uintah Basin District Health Department urges parents to make sure their children are fully vaccinated. Under the Utah School Immunization Law, "any pupil entering school for the first time in the state of Utah shall be required to present to the school one of the following: (a) a certificate (in this case the Utah Certificate of Immunization) Im-munization) from a licensed physician, clinic, or health department that the pupil has received or is in the process of receiving immunizations against those diseases specified by the State Division of Health or submission of evidence that the immunizations have been performed; (b) a certificate from a licensed physician stating that the condition of the pupil is such that any of the immunizations would be medically inadvisable; (c) a written statement signed by at least one parent or legal guardian that the immunizations violate religious or personal beliefs of the parent or legal guardian." Furthermore, Fur-thermore, "every school within this state shall provide the division of health with the name and address of any pupil who has not fulfilled the above requirements." Parents or guardians, it is your responsibility to ensure that your children have received the required immunizations, and you have submitted sub-mitted a "Utah Immunization Certificate," Cer-tificate," properly filled out before or at school registration. By October 1, 1979, any child who has inadequate vaccinations will be excluded ex-cluded from school until parents can produce written evidence that their child has had all his or her immunizations, im-munizations, or are in the process of completing them.
