Show NOTICE TO CONTRACTORS Sealed proposals will be received by the Utah Department Depart-ment of Transportation, Room 627, State Office Building, Salt Lake City, Utah, until 2 o'clock p.m. Tuesday, Feb. 7, 1978, and at that time publicly opened for construction of One Single Span Prestressed Concrete Con-crete Structure 104.88' 0. to 0. And a Major Concrete Box Culvert in Grand County, the same being Situated on U.S. 163 at Valley City Wash and Little Valley Wash identified as State Project No. NF-13-2(6) The principal items of work are approximately as follows: Portland Cement Concrete Class "AA" (AE) (Estimated (Esti-mated Quantity - 480 Cu. Yds.) at Per Lump Sum, 4 Each Prestressed Concrete Member (Type V) 103'-4 Long (Specialty (Special-ty Item) and 471 Lin. Ft. of Piles (Other Than Timber) 2'-6" Diameter. The project is to be completed on or before June 15, 1978. Prequalification of bidder is required. Prior to any bidder's receiving plans and specifications, it will be necessary that the bidder have on file with the Utah Department Depart-ment of Transportation completed com-pleted and approved, the required Prequalification Statement pertaining to contractor's con-tractor's experience and financial finan-cial condition. Not less than the general prevailing wage rates as determined by the Industrial Commission of Utah, effective April 20, 1976, shall be paid to all laborers, workmen and mechanics employed on this project. Proposal forms, plans and specifications are on file in the office of the Utah Department Depart-ment of Transportation, Salt Lake City, and the office of Its District Director at Price, Utah where they may be reviewed by prospective bidders. Specifications, Spec-ifications, proposal forms, and plans may be obtained only at the Department Offices at Salt Lake City, upon application and payment of a fee of $5.00, no part of which will be refunded. Each bidder must submit a bid bond from an approved surety company on forms provided by the Department, or in lieu thereof, cash, certified check, or cashier's check for not less than 5 of the total amount of the bid, made payable to the Utah Department of Transportation, as evidence of good faith and a guarantee that if awarded the contract, the bidder will execute the contract and furnish the contract bonds as required. The right to reject any or all bids is reserved. Any additional information may be secured at the office of the Department of Transportation. Transpor-tation. Dated this 21st day of January, 1978.
