If you took your first eye test when the law first became effective. Mav 10. 1955 then you must have an-other eye test before May 10, 1960. You must take this test dur-ing the three months of the end of the 5 year period. If you should ask to take it be-fore the last three month per-iod you will be charged with the cost of another license; but if you take it sometime during this last three month time there will be no cost to you. So the thing to do is look at your drivers license now for the date of your last eye test, add 5 years to that then get to Provo within the last three month period of the last year. In lieu of submitting to an eye test by the Department, the law permits holders of licenses to furnish the Department, in person or by mail a doctor's certificate showing their vision corrected or uncorrected to be 20-4- 0 or better. If the license holder mails an eye test certi-fication to the Department, it will be necessary for him to also send in his driver's license in order that the department may properly stamp and return the license. If you are unable to make out the eye test date due to wear or fading of ink, ,you may determine the correct date by directing an inquiry to the Driver's License Division, 123 State Capitol, Salt Lake City 14, Utah. Be sure to fur-nish your name as shown on your license, the license num-ber and your present address.
