According to a Utah Highway Patrol Trooper David Excell, JoAnn W. Slade. 41, Blanding, was pronounced dead at the scene of the accident about one mile north of here on Interstate 15. Her daughter, Jennifer, 18, was in serious condition in the Valley View Medical Center, but has since been released. Two other daughters, Diane, 20, and Tycie, 3, sustained only minor injuries.
