C, that Form 1040, for incomes of more than $5,000, for 1921, will be supplied to him by January Jan-uary 30th, on which date they may lw obtained from tho main office in .,-,. ai Lake City, and the zone offices T at Ogden, Logan, Price, Richfield, f and Milford. I'br the convenience of the public a copy of Form 1040 will be mailed to every taxpayer who filed a 1040 1 return last year. Others who nced , them will be furnished upon npplica- t tion.
