VA pension questionnaires Veterans and dependents who receive Veterans Administration Ad-ministration pension benefits can expect an annual income questionnaire in the mail this month, according to the VA Regional Office in Salt Lake. The questionnaire cards must be completed and returned by Jan. 1 or benefits will be automatically suspended. Beneficiaries over age 72 and those on the pension rolls for two years will also receive questionnaires this year. Anyone needing assistance in completing the questionnaire can visit the regional office at 125 South State Street, Salt Lake City, or telephone 524-5960. Applicants must be Chamber Bureau members and have served on a ChamberBureau committee during the past year. Defensive driving classes Utah's defensive driving course is a continuing campaign of the Utah Safety Council to reduce highway deaths, injuries and property damage by educating drivers in defensive driving techniques. It is an eight-hour classroom course on professional techniques for crash avoidance.
