O Breast cancer revisited: New therapies and enhanced mammography screenings Part Two In Part One of this series on breast cancer therapy, I wrote about breast cancer stages and the new drug therapies that are being developed that will change the way breast cancer is treated. These drugs are currently being used in clinical trials around the country and internationally. A new use for the drug Evista, a relatively new hormone-replacement drug is reported in the Journal of the American Medical Association (January 2002). Evista has previously been used as a substitute sub-stitute for estrogen-replacement therapy in menopausal women. Evista works by blocking the effects of estrogen, or estradiol, in the body. Many doctors are prescribing pre-scribing this drug to prevent osteoporosis osteo-porosis (thinning of the bones) and to possibly protect the heart It does not prevent hot flashes. In a recent study at the University of California, San Francisco, 7,200 women with osteoporosis were tested for estradiol levels. They found one-fifth one-fifth less new breast cancers in the Evista group than the women in the placebo group. The researchers in this study conclude that Evista works to reduce breast cancer in women with high levels of estradiol in their blood. The same did not hold true for women with low estradiol blood levels. These women already had a low risk for getting breast cancer and Evista did not lower the risk any further. The lead author, Dr. Steven Cummings, and his colleagues col-leagues in this study showed that women with high estradiol levels who took Evista for four years lowered their risk for breast cancer by 47 percent This is encouraging news and should be studied further over a longer period of time. Another exciting breakthrough is the result of the first research comparing an established antinausea anti-nausea drug with acupuncture in preventing nausea and vomiting. This may be used either before or after breast surgery, or during a course of chemotherapy. The study was presented in October 2001, at the annual scientific sessions of the American Society of Anesthesiologists in New Orleans. In spite of the fact that acupuncture acupunc-ture has been used for over 5,000 years, Western medicine has been reluctant and slow to accept this "alternative" therapy. In this study researchers compared com-pared the effectiveness of the well established anti-emetic (anti-vomiting) Zofran to acupuncture. They found that acupuncture was not only as effective but offered other advantages as well. Unlike anti-sickness, anti-sickness, acupuncture is more cost effective-and does not have any side effects, according to lead researcher Dr. TJ. Gan at Duke University Medical Center. I" yd , V common for women who undergo any breast surgery such as breast augmentation, reduction or mastectomy mas-tectomy to experience nausea and vomiting. In the Gan study, 40 women who were undergoing breast surgery with anesthesia were enrolled. The women were divided into three groups. Prior to the surgery, one group received acupuncture, the second an antiemetic anti-emetic drug, ondansetron, and the third a placebo. Only 23 percent in the acupuncture group reported nausea and vomiting two hours after the surgery, compared to 36 percent in the drug group, and 69 percent in the placebo group. Even more dramatic was the finding that only 38 percent of the acupuncture group reported nausea after 24 hours following surgery. This is versus nausea in 57 percent and 61 percent in the drug and placebo groups, respectively. The researchers also found that both acupuncture and the drug were equally effective in relieving nausea and vomiting following surgery. However, the acupuncture group had significantly less vomiting vomit-ing than the other two groups the day after surgery. Pain reduction was also a benefit. Only 31 percent of the acupuncture group reported pain compared to 64 percent in the drug group and 77 percent in the placebo group. It should be noted that the investigators in this study did not use the traditional acupuncture method. Rather, they used elec-troacupuncture. elec-troacupuncture. In this method an electrode similar to those used in an EKG test was used on the points of the body thought to relieve nausea and vomiting. This method delivers an electric pulse through the skin. The points used in the study were on the palm of the hand. The basis of this study will be the focus of a larger clinical trial sponsored by the National Institutes of Health (NIH). Daniel Sessler, M.D., an expert in acupuncture research and Associate Dean at the University of Louisville, Ky., says that in more than 6,000 existing studies using acupuncture, only a handful reported scientifically sound and valid findings. He concludes however, how-ever, that the Gan study has excellent excel-lent methodology.
