When individuals in-dividuals request a name change on a driver license, they must now show a legal document, such as a marriage license, or Court Order verifying that they have legally changed their name. This policy change is intended to ensure the accuracy of name changes ton driver licenses for use by law enforcement agencies and for those who use a driver license as a means of identification. Individuals with less than 50 points on their driving record are eligible for renewal by mail. However, if your name or address has changed and you have not received a renewal application, ap-plication, you may do one of two things: Apply at any Driver License Field Office and renew your license through normal testing procedures; or contact the Driver License Services Central Office at 533-5149 in Salt Lake City and inquire as to why you have not received the Renewal-by-Mail Notification. If your application was mailed to the last known address, you must go to one of the Field Offices and renew your license through normal tesUng procedures. If your Application was not mailed, the Driver License Services Office will supply you with an application and a 30 day temporary extension of your driver license to allow you time to complete com-plete your renewal. If you have any questions concerning these policy changes, feel free to contact any of our offices for assistance.
