The OgdenHAFB center will be closed Dec. 23, 26, 27, and 28, 1 988. Registration at Hill AFB Center begins: November 21, 1988. Registration at downtown Ogden Center begins: November 21 , 1 988. Call 750-1666 for information. The Bookstore is open Monday through Fridays from 8 am to 5 pm and on Saturdays from 10 am until 2 pm. The bookstore will be open January 3, 4 and 5 from 8 am to 7 pm. CONTINUOUS GRADUATE REGIS- TRATION: All graduate students j must be in continuous registration j until they complete all require- j ments for the degree. Students must be regularly enrolled for a minimum of three credits per j quarter (summer quarters j excepted). To meet this minimum, j students may register for regular ! courses or for 699 and 799 (Contin- j uous Graduate Advisement). j LATE FEES: TuitionFees are due ! before the first day of the term or quarter. A $10 late fee for all ; classes will be assessed beginning the first day of classes. This applies to all students, graduate and undergraduate, military and civil- , ian and also applies to students on VA benefits. A $20 late fee will be assessed beginning the second week of classes. WITHDRAWALS: If you plan to withdraw from a class, you must notify us and complete a drop form. An "F" grade is recorded on students transcripts when classes are not properly dropped. Drops cannot be accepted after specific dates. Military students going TDY must make arrangements to drop classes prior to leaving. A signature is required on a dropadd form. No drops can be accepted over the phone. INCOMPLETE: Students who must drop a class may with instructor permission receive an Incomolete "F" grade on the transcript, and have one year to repeat the course. (Military students with Tuition Assistance have only 6 months to complete an Incomplete course without reimbursing the Government.) Govern-ment.) Students do not have to pay for the course during the quarter I the remaining course requirements are completed. Students must inform in-form one of the Ogden Centers when they plan to complete the Course. A Change of Grade Report Form must be completed by the instructor. FINAL GRADES: Final grade reports C are sent out to students from the Hf USU campus within 30 days of the final exam. Privacy laws will not allow USU Center personnel to give j grades over the phone. Students requiring final grades before they receive them in writing from the campus should contact their instructor in-structor or the Office of Records on campus. Official transcripts may be requested by writing the Office of Records, Utah State University, j Logan, UT, 84322-1600. A $2.00 fee 1 will be charged for the first and second copies and a $1 .00 fee for all others on the same order. IT IS THE STUDENT'S RESPONSIBLE RE-SPONSIBLE TO CHECK FOR COURSE PREREQUISITES PRERE-QUISITES AND OTHER USU POLICIES. Individuals not employed on Hill AFB may enroll in classes offered on the base. Classes may be canceled end instructors reassigned before the first class session if students do not register during scheduled registration. OFFICE HOURS: The HAFB office is open Monday through Thursday from 8am to 5pm and Friday from 8am to noon. The Oqden Onice is open Monday through Friday from 8am to 5pm. Both offices are closed from noon to 1pm for lunch. Offices are sometimes temporarily closed in order to go to the bank or run other Center related errands. Seventy-five dollars ($75.00) of the total must be paid at registration and a $22.00 finance charge will also be added to the total amount due. If your tuition and fees will be paid by a company or institution, it will not be necessary to pay the $75.00 if the registration is accompanied by an official authorization of billing. COMPUTER LAB FEES: There is a $20.00 lab fee for each computer class requiring a lab each quarter. Students may use their own computers, com-puters, but must pay the $20.00 lab fee even if USU computers are not used. Students using USU. computers com-puters must also buy computer time. The time is available in $5.00 increments. It will not be possible to refund the computer time fee of a student deciding to use hisher computer after the computer time has been logged on the VAX computer. Each student must complete com-plete a computer usage card at the time of registration in order to receive a user name and password in time for the starting of the quarter. CIVILIAN JOB-RELATED NON-DUTY HOURS TRAINING: Civilian employees assigned to Hill AFB may be eligible to receive assistance with 50 percent of tuition costs for ONE job related class per FISCAL quarter (1st quarter, October-December; 2nd quarter, January-March; 3rd quarter, April-June; and 4th quarter, July-September). LIBRARY MEDIA ENDORSEMENT OFFERED IN OGDEN USU will be offering the Library Media Endorsement (required by the Utah State Office of Education) in Ogden on Hill AFB for persons supervising, directing or coordinating media services at the elementary or secondary (K-12) levels. The Library Media Endorsement En-dorsement is required of all new personnel entering the library media field. To be eligible for the Certification, students must hold or be eligible to hold a Basic Professional Pro-fessional Certificate with an Early Childhood Elementary, Middle, Secondary or Special Education Endorsement.
