The babies were mistakenly given 1,000 times a prescribed dose of a blood thinning drug, Heparin. Hepa-rin. The Quaid's counted the hours as their tiny infants struggled to recover from this life-threatening overdose. over-dose. Caused by an unintentional unintention-al but preventable medication medica-tion error, this scenario is repeated re-peated far too often across the country, despite the best intentions of healthcare providers. According to the Institute of Medicine, 1.5 million patients suffer each year from mistakes with the' medicine they're given and as many as 7,000 Americans may die annually from preventable pre-ventable medication errors. Mountain View Hospital Hospi-tal was among the first in Utah to take action against accidental overdoses and other medications errors by implementing an advanced technology called the eMAR (electronic medication administration ad-ministration record). This innovative and comprehensive compre-hensive system has been protecting hospitalized patients pa-tients for several years now. Many eMAR systems use the same bar code scarming technology found in grocery store checkout lines. But, the familiar groups of lines, dashes and numbers now help to prevent inadvertent human error when medications medica-tions are administered to hospitalized patients. Therefore, pharmacists, clinicians cli-nicians and nurses at Mountain Moun-tain View Hospital use the eMAR system 24 hours a day, seven days a week to significantly reduce the risk of medication errors and adverse ad-verse drug events. At Mountain View, medications medi-cations are scanned three-to-four times as they are distributed from the hospital pharmacy to each patient's bedside. Before a medication medica-tion is administered, care givers scan bar codes on the container ana tne patient s wristband, comparing both to the information in the patient's electronic medi cal chart. This systematic process alerts caregivers to potential errors and makes sure that the prescribed medication is given to the right patient in the correct dose.
