Proposal forms, plans, and specifications are on file in the office of the Utah Department De-partment of Transportation, Salt Lake City, and the office of-fice of its District Director at Cedar City, Utah where they may be reviewed by prospective bidders. Specifications, Speci-fications, proposal forms, and plans may be obtained only at the Department Offices Of-fices at Salt Lake City, upon, application and payment of a fee of $20.00, no part of which will be refunded. Each bidder must submit a bid bond from an approved approv-ed surety company on forms provided by the Department; or in lieu thereof, cash, certified cer-tified check, or cashier's check for not less than 5 of the total amount of the bid, made payable to the Utah Ut-ah Department of Transportation, Transpor-tation, as evidence of good faith and a guarantee that if ' awarded the contract, the bidder will execute the contract con-tract and furnish the contract bonds as required. Prequalification of bidders bid-ders is required. Prior to any bidder's receiving plans and specifications, it will be necessary that the bidder have on file with the Utah Department of Transportation completed and approved, the required Prequalification Statement pertaining to contractor's experience and financial condition. The attention of bidders is directed to the Special Provisions Pro-visions covering subletting or assigning the contract. In conformity with the Federal -aid Highway Act of 1968 the U.S. Department of Labor has certified the wage rates for this project which are the minimum wages to be paid on this project. These rates are made apart of the contract documents. The attention of bidders is directed to the fact that this Department has been advised by the Wage and Hour Division, Di-vision, U.S. Department of Labor, Jhat contractors en gaged in highway construction construc-tion work are required to meet the provisions of the Fair Labor Standards Act of 1938, (52 Stat. 1060). The Utah Department of Transportation, in accordance accor-dance with the provisions of Title VI of the Civil Rights Act of 1964 (78 Stat. 252) and the regulations of the Department of Commerce Com-merce (15 C. F. R. The length of road to be constructed or improved is 4.644 miles, and the principal princi-pal items of work are approximately ap-proximately as follows: 51,-400 51,-400 C. Yds. of Recycled Asphalt As-phalt Concrete Pavement, 27,800 Tons of Untreated Base Course 34"Maximum and 236,000 Sq. Yds. of Remove, Re-move, Crush and Stockpile Existing Bituminous Pavement.
