Show SUMMONS AND NOTICE TO PARENTS OR GUARDIAN BY PUBLICATION Case No. 31512 In the District Juvenile Court for Salt Lake County, State of Utah STATE OF UTAH, in the Interest of: BABY GIRL PRUETT 2 Mos. (5-30-62) Alleged neglected child. TO WILLIAM M. PRUETT, Father of the above-named child WHEREAS a Petition having been filed in this Court alleging the above named child is neglected, neg-lected, in substance as follows: (1) That on the 31st day of May, 1962, Peggy Ann Pruett, mother of said child, did execute a certain document with the Children's Aid Society of Ogden, a licensed adoption agency, and as required by law released all of her parental rights regarding said child and did authorize and request that the aforementioned agency place said child in a suitable suit-able home for the purpose of adoption. (2) That William M. Pruett, former husband of said mother, is not the natural father of said child and the purported natural father is a man named "George," and that William M. Pruett is the legal father of said child. (3) That the legal father and the natural father of said child have both neglected and refused to provide proper and necessary subsistence, education, medical and surgical care and other care necessary for said child's health and well-being. (4) That said child has been abandoned by its parents. (5) That William M. Pruett should be permanently and completely com-pletely deprived of all right, title and interest in and to the care, custody, control and guardianship guard-ianship of said child and that said child should be placed for adoption with the Children's Aid Society of Ogden. NOW, THEREFORE, you the father of the above named child, are hereby summoned to appear before the above named Court in said County on the 7th day of September, 1962, at 9:30 o'clock in the Courtroom of this Court located at 504 City and County Building.
