Show NEW PROGRAM TO INCREASE ACCURACY OF BIRTH CERTIFICATES The birth certificate of every baby born in Utah is kept permanently per-manently in the division of vital statistics of the State Department j of Health. j As the babies mature in years, I they will be called upon many times to show proof of age, birthplace, birth-place, and parentage. The emergency emer-gency during the past few years has shown the value placed upon birth certificates. Mrny of the certificates of birth which are filed in the state division divi-sion of vital statistics are believe be-lieve 1 to contain statements which are in error, or to be incomplete. In o der to determine to what extent ex-tent this is true, a birth query form has been prepared for mailing mail-ing to parents of newborn babies. This form will show a copy of the child's birth certificate as it is on file in the division of vital statistics. sta-tistics. It will be sent soon after the certificate has been received in the state office. A postage-free postage-free envelope will be enclosed for use in returning the form. Many persons, when obtaining their birth certificates, have found that certain items of information were given incorrectly, or omitted altogether on the original certificate. certifi-cate. Frequently, documentary doc-umentary evidence must be obtained ob-tained and presented in support ot additions to the record. It is hoped this new plan will elim'nate these errors and thereby increase the division's service to the people of Utah. |
