In addition, families not meeting these rriteria but with unusual expenses due tu unusually high medirnl expenses, shelter costs in excess of 30 percent of income, xiei-iu-l eduration expenses due to the mental or phyxiral condition of a child, and disaster or casualty losses are urged to apply. Application forma are being sent to all homes in a letter to parents. Additional eopies art available at the princiwl'i office in rarh school. The informstion provided on the application is confidential and will be used only for the purpose of determining eligibility. Applications may be submitted at any time during the year. In certain cases, foster rhildren are a.lso eligible for these benefits. If a family has foster rhildren living with them snd wishes to apply for sueh meals and milk for them, it should contact the school. Under the provisions of the policy, the Principal of the school will review applications and determine eligibility. If a parent if dissatisfied with the ruling of the official, he may wish to discuss the decision with the determining official on an informal basis. If he wishes to make a formal appeal, he may make a request either orally or in writing to Superintendent Thomas J. Abplanalp for a hearing to appeal the decsion. The policy contains an outline of the hearing proceed u re. If a family member becomes unemployed or if family size changes, the family should contact the school to file a new application. Such changes may make the children of the family eligible for reduced prire meala, or for additional benefits surh as free meals and milk if the family income falls at or below the levels shown above. In the operation of child feeding programs, no child will be discriminated against because of rare, sex, color or national origin. Each school and the office of the Duchesne County School District has a copy of the complete policy whirh may be reviewed by any interested party. Children from families with a yearly gross income from all sources at or below the following levels should be eligible for free meals or reduced price meals and free special milk.
