Show REGISTRATION It is the student's responsibility to check for course pre- requisites and other USU policies. Students may register as soon as they receive this bulletin. Payment must accompany registration. In order to avoid the late fee, students must register in person by September 28, 1994. BOOK BUY BACK The USU Bookstore on campus will buy back books dur6 for ining final exam week each quarter. Call formation. The Bookstore is open Monday through Friday from 8 a.m. to 5 p.m. 797-166- USU LIBRARY SERVICES Merrill Library (located on the USU campus) now has a Distance Education Library Service Program which provides materials and informational support for all USU centers. For more information, please call 1 or (only within Utah). 621-586- CANCELING OF CLASSES Due to instructor staffing needs, book ordering deadlines, and other requirements, students should register before classes are to begin. Classes may be canceled and instructors reassigned before the first class session if students do not register during scheduled registration. ID CARDS Students will need an ID card in order to check books out of the libraries of the institutions of higher education. No fee will be charged for a student ID. LATE FEES TuitionFees are due by the first day of the quarter. A $20 late fee for all classes will be assessed beginning on September 29, 1994. This applies to all students; graduate, undergraduate, military, civilian, and students receiving VA benefits. WITHDRAWALS If you plan to withdraw from a class, you must notify us and complete a drop form. An "F" grade is recorded on students' transcripts when classes are not properly dropped. Drops cannot be accepted after the specified date. Military students going TDY must make arrangements to drop classes prior to leaving. A signature is required on dropadd forms. No drops can be accepted over the phone. INCOMPLETE Students are required to complete all courses that they are registered for by the end of the quarter. In some cases. a student may be unable to complete all of the work in a course due to extenuating circumstances, but not due to poor performance. Such a student may petition the instructor of the course for time beyond the end of the quarter to finish the work. If the instructor agrees, two grades are placed on the final grade list for the student - an "I" and a letter grade for the course is computed as if the missing work were zero. The student is then required to complete the work by the time agreed upon, a period of time no longer than 12 months. If no change of grade has been submitted by the instructor within the prescribed time period, the "I" grade will be removed and the letter grade originally submitted with the "I" will remain as the permanent grade for the course. also enroll in concurrent education classes through ext. 610 for inlocal high schools. Call formation. FINAL GRADES GRANTS Final grade reports are sent out to students from the USU campus within 30 days of the final exam. Privacy laws do not allow USU SUCCE personnel to give grades over the phone. Students requiring final grades before they receive them in writing from the campus should contact their instructor. Official transcripts may be requested by writing the Office of Records, Utah State University, A $3 fee will be charged for Logan, UT, 84322-160each official transcript and $.50 for each unofficial tran- students taking classes Grants are available for part-tim- e off campus. For information, call the Financial Aid Office at (Ask about Pell Grants and others.) script. CLASSROOMS CHARGE CARDS ACCEPTED Please help us keep the classrooms free of litter, paper, etc., and in good operating condition for other students using the facility. 0. At this time, tuition can now be paid for with your Visa or MasterCard. For further information, please contact the SUCCE Office at ext. 610. 637-212- 0, DEFERRED FEE NOTE If you are unable to pay the total amount of your tuition at one time, a Deferred Fee Note may be completed. The remaining tuition and fees are deferred to payment no longer than 60 days from the first day of the quarter. One hundred twenty-fiv- e dollars ($125) of the total tuition dolamount must be paid at registration and twenty-tw- o lar ($22) finance charge is added to the total amount due. All previous deferred fee notes must be paid in full before a new form can be completed. If your tuition and fees will be paid by a company or institution, it will not be necessary to pay the $125, if the registration is accompanied by an official authorization of billing. (Deferred Fee Notes cannot be used for audit classes or books.) All students using tuition assistance that includes the cost of books must pay in advance for the books. Once TA monies have been received by the USU campus the book costs will be returned to the student. HIGH SCHOOL SENIORS Senior students may enroll for Independent Study classes. The Independent Study catalog may be picked up at the SUCCE Office. Senior students may 2 637-212- 0, VETERANS Please note - Veterans who have not paid their tuition and fees in full, two weeks after the quarter begins, will be notified that they will be terminated from the VA. (This applies to those on GI Bill) Veterans should call Jo Ann 2 Toone at for information before registering. Servicemen using Military Tuition Assistance must bring 797-110- a copy of the form with them 797-017- to registration. 3. AUDIT CLASSES Classes taken for credit and audit are recorded on an official Utah State University transcript. Audit classes do not receive a letter grade and cost the same as for credit. CLASS CHANGES Please bring your registration receipt with you when making any class changes. No refunds or class changes will be made without your receipt. (Students must accept responsibility to become aware of USU policies and dates as provided in the current catalog.) If classes are canceled, an adddrop form must be filled out by students and returned to SUCCE by end of the first week of class. SYSTEM COM-NE- T Some of the classes being held at SUCCE are being delivered over a telecommunication system. There are about 1 1 other Corn-Nlocations connected to the system. Two way communication between the student and instructor is possible by using microphones electronic blackboards, video slow-sca- n cameras, and a fax copy machine. Only two students per location are required in order to carry a Corn-Nclass. This means few classes are cancelled. For individuals on TDY schedules this is a good educational alternative since class sessions can be taped.
