Two deaths and 14 other instances in which the slotted hangers became unhooked, along with 17 instances of plastic brackets breaking or bending, have been reported to the CPSC. If the hanger becomes unhooked or a plastic bracket bends or breaks, the mattress can tilt down when the child moves to the corner. The child can then slide down and become caught between the mattress and the side rail or end panel of the crib. In 19 of the reported incidents, including the two deaths, the children became entrapped between the mattress and the crib. The CPSC believes the hangers may become unhooked when the crib is moved, the bed is made or 1979 and but the 1981, CPSC port hangers. The fans offered by AAFES involved in the recall are Model CF4152 in white, CF4152 in brown and CF4352 AB in antique brass. All these 1985 series fans sold for $92.95 in exchange stores. Only fans produced during April 15 to April 25 are involved in the recall. The potentially defective fans will all have the letters "IFD" in the first group, meaning that the fans were made in April 1985.
