Making a substantial contribution to the improvement of human relations, employee motivation, or morale of a large or important organization. Management of an activity for a sustained period (one year or longer) in a manner clearly exceptional in comparison with others doing a prominent job in a similar activity or activities of similar scope of responsibility. Evidence of outstanding success in representing AFLC in pursuance of the command's mission, in association with individual industries, industrial organizations, general public, Hq USAF, DOD, etc. Nominations should be submitted on AF Form 642 "Recommendation for Decoration," to include citation in original and six copies and should contain the following: Description of achievements in narrative form, carefully documented. Attachments may be added if they contribute to better understanding of achievements. Brief, factual biographical report in narrative form not to exceed one typewritten page. Three 8 x 10 inch photographs. Citation, not to exceed 70 words. Statement by the nominee indicating" whether or not he has any objection to eommand- wide releases of publicity if granted an award.
