Show BIRTH QUERY BEING SENT TO PARENTS The birth certificate of every baby born in Utah is kept permanently perma-nently in the Division of Vital Statistics of the State Dept. of Health. As the babies mature in years, they will be called upon many times to show proof of age, birthplace, birth-place, and parentage. The emergency emer-gency during the past few years has shown the value placed upon birth certificates. Many of the certificates of birth which are filed in the State Division of Vital Statistics are believed be-lieved to contain statements that are in error, or to be incomplete. In order to determine to what ex-ten ex-ten this is true, a birth query form has been prepared for mailing to parents of new-born babies. This form will show a copy of the child's birth certificate as it is on file in the Division of Vital Statistics. Sta-tistics. It will be sent soon after the certificate has been received in the state office. A postage-free envelop will be enclosed for use in returning the form. The parents are requested to examine the copy of the birth certificate cer-tificate as it was filed for errors or omissions. If it is found to be completely and accurately filled out, the parent should sign the form and return it to the state office. of-fice. If there are any corrections or additions to be made, they should be entered on the form by the parent and returned to the state office where the corrctions will be made on the original certificates. cer-tificates. Many persons, when obtaining their birth certificates, have found that certain items of information infor-mation were given incorrectly, or . omitted altogether on the original certificate. This has caused them inconvenience and sometimes considerable con-siderable expense. ' Frequently, documentary evidence must be obtained and presented in support of additions to the record. It is hoped that this new plan, will eliminate these errors and thereby increase the Division's service to the people of Utah. |
