Show PANGUITCH CITY REQUEST FOR STATEMENTS STATE-MENTS OF INTEREST. QUALIFICATIONS, AND PERFORMANCE Panguitch City (CITY) desires to procure services within the scope of practice of architecture and professional pro-fessional engineering to prepare a Wastewater Facilities Plan in accordance accor-dance with 40 CFR 35 and State Department of Environmental Quality Qual-ity requirements and other City public works projects. The CITY is requesting qualified firms (FIRM) engaged in providing architect-engineer services to sub-mil sub-mil a statement of interest, qualifications qualifi-cations and performance data which should include, but not be limited lo the lollowing: (a) the name of ihc FIRM and the location ol all of its offices specifically indicaling the principal place of business, (b) the age of the FIRM and its average number of employees over the past five years, c) the education, training and qualifications of members of the FIRM and key employees, (d) the experience of the FIRM reflecting technical capabilities and nroicct experience. (e) the names of five clients who may be contacted, including al least two for whom services were rendered in ihe last year, (f) Planning, design and construction con-struction management experience on similar wastewater projects within the State of Utah. For each project listed, provide name and phone number of a contact person representing the client. Statements of Qualifications will be received until 5:00 p.m. on Augusi 20, 1993 at the Panguitch City Hall, 55 South Main. P.O. Box 77, Panguitch, Utah 84759. Six copies are required. The FIRM will be required to provide ihc scope of work outlined below in preparing the Wastewater Facilities Plan. The Plan will substantiate sub-stantiate the need for proposed facilities. facil-ities. The FIRM will prepare the plan through a systematic evaluation evalua-tion of alternatives that are feasible in light of unique demographic, topographic, to-pographic, hydrologic ancf institutional institu-tional characteristics of the Panguitch Pangui-tch area. The plan will demonstrate that the selected alternative is cost eflective, i.e., the most economical means ol meeting water quality and public health requirements over the design life of the facility while recognizing rec-ognizing environmental and other non-monetary considerations. The Plan will also demonstrate that the selected alternative is imple-mcntable imple-mcntable from legal, institutional, financial, and management standpoints. stand-points. Criteria to be used in evaluating the statements of interest, qualifications, qualifica-tions, and performance data and selecting se-lecting FIRMS shall include: (a) competence to perform the services as reflected by technical training and education, general experience, ex-perience, experience in providing the required services and the qualifications qualifi-cations and competence of the persons per-sons who would dc assigned to perform per-form the services. (b) ability to perform the services ser-vices as reflected by workload and the availability of adequate personnel, person-nel, equipment, and facilities to perform per-form the services expeditiously, and (c) past performance as reflected by the evaluations of private persons per-sons and officials of other governmental govern-mental entities that have retained the services of the FIRM with respect re-spect to such factors as control of costs, quality of work, and an ability abil-ity to meet deadlines. The successful linn may be selected se-lected solely from the written statement of interest, qualifications and performance data; or no more than two or three firms may be invited in-vited to discuss with the CITY each FIRM'S general capabilities. After review of written, statements ol qualifications and if required subsequent subse-quent to discussions with FIRMS, the CITY shall select and rank in order of preference the firms which it deems to be most qualified to provide the required services. J The submitted statements are to be general in nature and no cost data is to be included. A cost proposal will be solicited from the top ranked FIRM after the selection process is complete. The CITY shall negotiate a contract with the most qualified FIRM for the required re-quired services at compensation determined de-termined to be fair and reasonable, taking into account the estimated value, scope, complexity and nature of the required services. If a contract cannot be agreed upon with the most qualified FIRM, the CITY shall enter into negotiations with the next most qualified FIRM, and so on, until a contract can be reached. At the successful completion of the planning phase of the wastewater wastewa-ter or other projects, the CITY may, at its own discretion, negotiate negoti-ate with and award the detail design phase and construction inspection phase for the project to the FIRM selected in this olfcring.
