Show - 20 A p ,V 1573 MAY DESERET NEWS, WE- - Enzyme treatment replaces surgery for back problems surgeons try to treat backaches conservatively, trying to relieve the pain without turning to surgery. CHICACO (AP) About a quarter of all Americans have bad enough backaches to go to a doctor about them. For a small fraction, surgery is required, sometimes involving fusion of a portion of the spine. . Were hot fusing nearly as many spmes as formerly, Wiltse said, although he added that some orthopedic surgeons might disagree. Fusion involves traasplanting part of the ilium found in the hip bone, or of the tibia, the calf bone, to the spine to stabilize a diseased or injured portion. Artificial devices, such as metal rods or plates and springs, also may be used. Dr. Paul R. Meyer Jr. a member of the Northwestern University Medical School faculty and director of the spinal cord injury unit at Northwestern Memorial Hospital, said spinal fusion today is most commonly used to correct idiocurvature of the spine from an unknown pathic scoliosis cause. This occurs most often in young women. Scoliosis at one time also was associated with polio, but that disease has been virtually wiped out. Over the past few decades, the time in bed required has dropped for some patients to a day or so, compared with the two or three weeks or even months they formerly were kept down. The time still varies, however. Those who require more extensive fusion have, of Involving several vertebrae course, to say in bed longer. There are indications that a recently developed enzume which can be injected in the spine for certain problems with the disks may further reduce the need for spinal surgery. This enzyme, called chymopapain, is derived from papaya. It is being used in only a few medical centers, pending further approval by the Food and Drug Administration before it is generally available. Dr. Leon L. Wiltse, a faculty member at the University of California Medical School at Irvine and staff member at Long Beach Hospital, said in an interview that orthopedic NASA sure M. 4 Astronauts can do space job When it does, it often can be treated successfully with aspirin and exercise and only occasionally requires fusion. Fusion also is done for the slipped disk or ruptured disk." Meyer said that perhaps only 2 percent of those who think their backache is caused by this problem actually have it. When it does happen, the fibrous, jellylike pad which separates the vertebrae slips or is forced out of its place. Pam results when this extruded mass causes pressure on a nerve. It is this problem which chymopapain has been shown to treat successfully. Another spinal operation, laminectomy, sometimes is used to relieve the pressure which causes pain. Meyer described this as unroofing of a disk or nerve root." The lamina, part of the vertebra, is removed in this procedure. However, spinal fusion sometimes is later necessary when this does not work. Both Wiltse and Meyer said that backache often is f psychosomatic complaint, caused by emotional problems But it still hurts just as much. Meyer outlined several other conditions for which fusion is sometimes required. Among them is degenerative arthritis. This occurs in everybody, starting in middle age, and does not cause any problem in most people. And both orthopedists cited the special problem o! workers who have back fusion. The back older heavy-dut- y often can- is never restored 100 percent, and these workers not return to their former jobs. and more meMeyer said improved surgical techniques ticulous surgery have helped make the fusion operation more successful and the length of time in bed shorter. for a fuGenerally, two or three months are required two or or.e segments sion to heal completely if it involves of bed of the spine, Meyer said, but the patient may be out in two or three days.
