Show Reflections, May 24, 1984, Page II house call By G. timothy Johnson, M.D. QUESTION: Exactly what is a hemorrhoid and what is the best way to treat it? I have been troubled by hemorrhoids since college and have put off having with a device specially designed for this purpose. When done by persons trained in the procedure, elastic band ligation has the advantage of avoiding more extensive surgical removal and, therefore, leads to a much quicker recovery period with less pain at a lower medical cost. the injection Sclerosing treatment of a chemical agent to harden the hemorrhoid is still quite popular; some physicians still advocate hemorrhoidectomy, the surgical removal of the hemorrhoid. If a hemorrhoid is strictly external that is outside and below the line dividing the skin from the internal mucosa of the rectum then different treatment approaches are typically utilized. Such external hemorrhoids are often anything done about them, other than treat the symptoms with some non- prescription medicines. ANSWER: We ' used to teach that hemorr- were nothing more than "varicose veins" in the area. rectal More modern therapy has it hoids that hemorrhoids are really mixtures of arteries and veins which would why explain bleeding from hemorrhoids is typically bright red, the color of oxygen-ricblood from the arteries, rather than dark red, the color of blood from the much more painful than internal hemorrhoids and they often can be cut out or cleaned out as an initial method of treatment. Obviously the treatment of hemorrhoids is not that simple and straightforward as one might guess. It is important for a knowledgeable physician to determine what treatment will work best for a given situation and, above all else, to treat in such a way that does more good than harm. Finally, I should point out that when an internal hemorrhoid falls out to the outside it is known as a prolapsed hemorrhoid; this situation may require h veins. Hemorrhoids also were typically classified into two groups: external (those outside the rectum, which could be seen) and internal (found inside the rectal canal or above the opening of the rectum). In either case, the treatments for hemorrhoids vary considerably in different parts of the world and from one doctor to another. Increasingly, however, the most popular treatment for internal hemorrhoids seems to be "elastic band ligation, meaning the tying off of the hemorrhoidal tissue with an elastic band which typically is inserted an entirely different treatment approach than those I mentioned for other hemorrhoids. The treatment goal here is to try returning the hemorrhoid to its internal location and then proceed with whatever treatment is petition 19 It. city 20 Ms. Golightly 21 Neighbor of Syria 22 Division word 23 Threw boxcars 24 Mideast deity 25 26 Thailand, s once 27 Marathoners theme song? 32 Long-taile- antelope 33 Except Theres Joy In The Natural And Redford Doesnt Strike Out The Natural. Directed by Barry Levinson. Screenplay by Roger Towne, Phil forfeited to heavy-dutallegory. One need only look to the Westerns of John Ford to see just how an archetypal character can possess both the stature of Monument Valley and the human accessibility of a John Wayne or a Jimmy Stewart. By failing to flesh out its characters, The Natural partially loses its footing in reality and its rapport with the audience. Yet, in the very obviousness of its symbolism, The Natural is, in effect, wearing its heart and pretensions on its sleeve. I had to respond to this ingen- Redford. The Natural rides on that very thin line between archetype and stereotype, frequently toppling from its high horse of symbolism into the allegorical muck below. Roy Hobbs (Robert Redford) embodies an American fable. Raised in . uousness as well as the undeniable talent, intelligence, and love at work here. The cast is stellar, though often misused, and these are the events that legends are made of: In an early scene, Roy strikes out the great "Whammer" (Joe Don Baker as an uncanny "Bambino") in an impromptu challenge at a small county fair. Sixteen ears later, when gruff coach Pop isher (Wilford Brimley) encourages Roy to knock the skin off the ball, he does exactly that. And when Roy takes that great homerun, under a hail of brilliantly radiant sparks, exquisitely shot by Caled Deschanel, he is being frozen in legend and memory by 50,000 human cameras. Starring Robert come the best baseball player ever. There is little doubt that Roy is, indeed, the best, performing quite like the robot in the "Mighty Casey" Twilight Zone episode, but he is constantly being detoured from his destiny by seemingly arbitrary and symbolically artificial obstacles that are placed in his path. Roy is loved by an angel (Glenn Close), but hounded by the forces of darkness: the dark Bird of Prey (Barbara Hershey), the golden seductress (Kim Basinger), the Gambler (Darren McGavin), and the Judge (Robert Prosky). The name of Roy's team, the New York Knights, embodies this conflict between heroism and shadowy evil. Unfortunately, depth of character is Containers 15 Saber com11 31 Duo possible.
