Do you have what it takes to file your federal income tax return? Employers are required to supply each of their employees with Form W-2, "Wage and Tax Statement," by February 1, 1994. If you don't receive a W-2 form by that date, ask your employer for one. Your employer can even provide you with a duplicate if your original is misplaced. If you cannot get the form by February 15, call the IRS toll-free telephone number for your area. The number is listed in the federal tax forms instruction booklets. You will be asked for your employer's name, address, telephone number, and if known, identification number (if avail- able, check your records for a prior year's W-2 form from the same employer).
