'l8 years 'old, died at Saltalr yesterday f hemorrhage of the brain shortly after leaving the lake. The young man had not been feeling well for several .days, but had1. not been confined to his room. He was advised by his physician phy-sician to take baths at the lake, and went out yesterday for that purnosA. While in the water he became ranldlv worse and shortly after coming to shore fell lri a faint. In a few moments he was dead. Walter Forman had been employed by the Oregon Short I vine. He was the son of Leonard and LiUte Grow Forman of Bingham Junction. He was born December De-cember 25, 1SS7. The remains are now at the undertaking room of Joseph William Taylor. 1 TRUST DEED IS FILED BY THE INDEPENDENT . A trust deed for $3.000,000,. covering a bond issue for that amount, by the Independent Inde-pendent Telephone company to the Utah Savings and Trust company was filed with the County Recorder yesterday. The bonds are to be 6 per cent gold bonds of various denominations running from 1 to G500. . The mortgage was made on August 6, IS16. and - acknowledged by lieber 1. Wells, president of the telephone company, com-pany, and Elmer B. Jones, the managing director, and John J. Daly, vice-president, and 8. H. Ivynch, secretary of the Utah Savings and Trust company. The mortgage mort-gage covers real estate in Salt Lake City, Ogden a lid various other cities in Utah, as well as franchises and other concessions conces-sions of value to the company.
