She was the beloved mother of Beverly Buy and DeLois Hale and grandmother of Jody Hale Holt, Jill Hale Hawkins, David C. Hale, and Ryan P. Buy, and greatgrandmother of eight. Hazel was bom on October 8, 1917 in Kanab, Utah to Frank Levi and Lillian Turner Farnsworth.
