Examples are croup, ear infection, sore throats, pneumonia, influenza (respiratory flu). Recognizing dehydration and assessing its severity is obviously central to coping with this problem. Besides knowing what illnesses can cause dehydration, there are specific signs which allow accurate assessment: 1. a frequency of urination less than 4 times in 24 hours. 2. dry mouth. 3. lack of tears when crying. 4. listlessness. 5. sunken eyes. ? 6. sunken soft spot (only in infants under one year of age in whom the soft spot on the top of the head is still not closed). These signs are listed in increasing severity. Generally parents will bring the infant into the doctor by step 4. Treatment is based on giving enough fluids of the right type to make up the total fluid deficit and to keep up with any ongoing losses. With stomach flu the best course is to stop solid food and milk for days (depending on the severity of symptoms), and to give large amounts of clear liquids (juices, KoolAid, Tang, soda pop, water, etc.), and then after the day period to slowly reintroduce solid food, formula, milk, etc. Usually the vomiting andor diarrhea will continue during the first few days, but the right techniques will allow the child to prevent or correct dehydration. The most important concept is to give Small, frequent amounts of liquids. This lessens the yomiting and allows the child to retain enough fluid to stop excessive fluid losses. If the diarrhea and vomiting are severe or last more than 2 days, it is important to give not only enough fluid, but also the right type. If a child is not eating, only drinking, as is usually the case with stomach flu, the sodium, potassium, chloride, and bicarb must be replaced (to make up losses in urine, diarrhea, vomit, and sweat). In cases where the cause of dehydration is not stomach flu, but merely decreased intake of food and liquid because of loss of appetite ( because of the main illness), the specific type of fluid is not so critical In these cases the fluid loss is not so great and often the infant or child continues to take some lood ( which supplies the above mentioned minerals and salts). Any fluid, even entirely water, will suffice in these less severe cases. Probably the best of all fluids to use in more severe cases, where the minerals and salts are being depleted, is Pedialyte, available in all drugstores and many grocery stores. It is specifically formulated to replace fluid and salts and should be used in the severe cases. A stratagem which sometimes works when the child refuses all other food and drink is popsicles or ice cream. To monitor progress and effectiveness of treatment, keep a log of the following items: total fluid intake per 24 hours, wet diapers per 24 hours, and temperature readings. These, along with exams and serial weights at the doctors office scales, will enable the physician and parents to confidently treat successfully the overwhelming majority of cases.
