A knife-lik- e pain I I shoots through your U back and renders you helpless. Backaches are perhaps the most common source of chronic pain. Sooner or later, four out of every five Americans will be forced off their feet by a problem with their stiff, aching backs. Backaches prompt more than 19 million doctor visits and strike about 8 million new victims a year. The cost of treatment, rehabilitation and time lost at work amounts to more than $14 billion a year. But now fast relief for the most severe bad backs is without painful possible or rehabilitation. long surgery Back injections that dissolve herniated or slipped discs may revolutionize the way crippling back pain is treated. The spinal discs are made of a tough, fibrous protein called collagen. This gelatinous material acts like a shock absorber between each pair of vertebrae, the bones that form the spinal column. When we say a disc slips or ruptures, it actually herniates or bulges out from between the spinal vertebrae and presses on the nerves coming from the spinal cord, causing pain. What causes a disc to deteriorate? We dont know the whole story, but its a degenerative process, a weakening in the disc space," says Charles Fager, M.D., chairman of neurosurgery at the 9 Lahey Clinic in Massachusetts. The disc loses its anatomical pattern and form and bulges out. Surgery has been the traditional way of treating disc injuries that dont respond to bed conservative measures rest, exercise and physical therapy. In an operation called a laminectomy, the surgeon removes the disc and fuses the spine together, a debilitating procedure with a long recovery time and no guarantee of success. Back injections, known as chemonucleolysis, offer a quicker, easier solution. The advantages are significant; generally, only a one-nighospital stay, plus less pain and disability compared to back ht surgery. So far, two drugs have been used in the injections, with varying results. The first is chymopapain, an enzyme derived from the papaya plant. Injected into the center of the disc, the enzyme dissolves the gelatinous material over the course of a few weeks. In our hands the success rate of chymopapain is above 80 percent, says Robert Baird, M.D., chief of orthopedic surgery at the University of California Medical Center, Irvine. I think chymopapain is equally good if not better than surgery, says Mitchell Krieger, M.D., an orthopedist at Michael Reese Hospital in Chicago. But there are many risks with it. There is a possible allergic reaction and there have been deaths from it." According to Mark Brown, M.D., of the University of ea. 1 Printed knits Suggested retail to $3.98 yd. But the cost is expected to come down. And compared to the $10,000 to $20,000 for back surgery not to mention the weeks of pain, incapacity and it may be rehabilitation worth the price. It breaks down the collagen in the disc, explains Dr. Brown, and it is not allergenic. The absence of allergic side effects is a plus for collagenase. There have been no deaths reported to date. Nor have there been relapses. We have never had a patient who gained relief from the injections who subsequently had a recurrence of back pain in the same disc, says John Bromley, M.D., an orthopedic surgeon at New York University Medical Center. Collagenase is still undergoing tests to determine its long-rang- e effectiveness and Because of the risk of complications, back injections must be given by certified Polyester Now available in 12 3.9 60" wide BULK SATIN RIBBON says. Dr. Fager disagrees. So far about 16 people have died from chymopapain and 12 have been paralyzed to one degree or another, he says.
