These questionnaires should be completed carefully showing all survivors of the deceased veteran, and returned to the Veterans Administration Center, P. O. Box 1260, Denver 1, Colorado. Based on facts obtained from these questionnaires and records on file, VA will determine who is legally entitled to the dividend. Application blanks for the dividend will then be mailed by VA to those eligible beneficiaries. In cases where the deceased veteran held two or more NSLI policies, the beneficiary may receive one or more dividend checks. This applies to beneficiaries in all three groups. In the event the first check received by the beneficiary does not cover the total dividend to which he is entitled, such beneficiaries are urged not to write the VA office immediately, as another check will follow at a later date.
