Last week the discussion focused on some of the general concepts of headaches and some of the structures which cause pain. This week will be the first article focusing on specific types of headaches. Muscle contraction or tension headaches are the most common of the various types of headaches. They are characterized by a steady aching, or tightness or pressure feeling around the head. They are almost always on both sides of the head usually beginning in the back of the head or the upper neck. As the name implies, it will often begin as a tightening of the muscles in the back of the neck which then spreads to the muscles around the scalp. It is sometimes described by patients as a bandlike constriction around the head that gets increasingly tighter. The muscle or contraction or tension headache may last a few hours or a few days or in some patients weeks, months or years. It is commonly associated with emotional stress and probably represents the physical response to anxiety, hostility or anger. In some cases sustained muscular V.A. The diagnosis is made purely on the clinical description information. historical and The physical examination and other evaluations are intended to eliminate other causes for such a problem. Management The management of this type of headache is directed in eliminating the factors which cause it in the first place. The mild varieties response to simple heat, ice bags or massage or the use of common pain medications such as aspirin. A cold washcloth applied to the base of the neck will often relieve the headache. Rest and relaxation are important components of healing the problem. On those patients in whom the headache begins as a result of emotional factors, attention to these factors and assistant in relieving the anxiety, stress, hostility or anger is what is needed to prevent fui ther attacks. Pain medicatio simply makes the person fee better, but does not eliminate th problem. It is important for the physicia. as well as the patient to realizi that people with these headache: are neither hysterical nor fakers They generally have a head that hurts, are in distress and are in need of help. However, many patients are not accepting of the fact that the therapy must be directed at the emotional problems that there may not be any physical reason for the headache. The diagnosis of muscle contraction or tension headache is not difficult when there is a clear relation between the onset of the headache and the emotional stress. When this relationship is not clear or there are other indications of other physical problems then a more thorough evaluation and examination is required. As mentioned previously, in many instances brain wave studies, brain scans and referral to a neurologist are necessary to reinforce the fact that no physical reason can be found for the headaches and it is the emotional factors which must be treated.
