The awards are designed to ognize individuals for outstanding support and contributions to the objectives of the Air Force Civilian Equal Employment Opportunity and Affirmative Action Program and provide visibility for the Air Force commitment to equal employment opportunity and affirmative action. Nominations cover the period Oct. 1, 1998 through Sept. 30, 1999. Submit an original and three copies of typed AF Form 741, Nomination for Distinguished EEO Award to the Ogden ALC Civilian Personnel Division by Wednesday. Also include a brief citation and a Privacy Act Statement, authorizing the release of personal data. The facts supporting the achievements or contributions of the nominees must be stated concisely, limited to the period covered and clearly define the results achieved in the specific category. Narrative justifications may be continued on plain bond paper, providing they don't exceed two additional typed pages. The Ogden Air Logistics Command Incentive Awards Committee will select one nominee in each category, which will be forwarded to Air Force Materiel Command. AFMC will approve one nominee in each category and forward them to the Air Force Level. mative action goals. Special Emphasis Programs, for civilian and military personnel who have contributed significantly to the employment and advancement of minorities or women. This Distinguished Award winners will receive Certificates of Achieve- ment and letters signed by the Secretary of the Air Force, along with engraved plaques. This award honors Air Force military and civilian personnel for dis- tinguished service, during the category includes five separate period covered, in support of the Equal Employment Opportunity and Affirmative Action Program. Categories are: Commander Action, open to commanders of major commands, awards: Federal Women's Program; Hispanic Employment; Black Employment; American Indian Alaskan Native Employment and Asian AmericanPacific Islander Employment Programs. Selective Placement Program, designed for civilian and military personnel who have made a significant contribution to the employment or advancement of disabled persons. separate operating activities, direct reporting units, intermediate com- mands and wing, base or tenant organizations.
