The survey will be randomly mailed to TRICARE Prime beneficiaries, and has the endorsement of medical personnel and the commander of Hill Air Force Base. The health survey takes only 15 to 20 minutes to complete. The survey will cover questions about health care systems, health status, health habits, tobacco use, alcohol use, diet, demographics, and military specific information. Your responses will remain confidential and results will only be reported in terms of groups. Although the survey is voluntary, you are urged to complete and return it to the University of Minnesota. fZ C.Ci |
