I was amazed when the video of 'he airliner floating in the river was aired on the news. The nation na-tion first gasped with horror, and then later sighed with relief when it was reported that all passengers and crew had survived sur-vived the emergency. The airline industry is a riMxIel for all when it comes to its record of safety. However, that's not always been the case. Years ago a huge change occurred oc-curred when every accident or near-accident event began to be critically analyzed to find the root cause of failure. Many procedural changes came from that process and have made air travel incredibly safe. Years ago, medical professionals profes-sionals became alarmed by their own safety record and repetition of complications. Physician and hospital leaders looked to the airline industry as consultants to help find answers and solutions to prevent errors. From that effort, a process similar simi-lar to the pre-flight checklist on airplanes has evolved to protect patients within hospitals. What resulted Is what we call the "Universal Protocol" for surgery sur-gery and other procedures. It involves a pre-surgical checklist to try to eliminate errors. It's called a "procedural timeout" and is a process designed to promote patient pa-tient safety during a surgery or invasive radiologic procedure. It occurs when a newborn is circumcised, when a patient has a heart catheterization, when an invasive radiologic procedure is done, and for every surgery in the operating room. Results of this study showed more than double the improvement that had been expected ex-pected at the start of the study. The most serious of these errors are procedures done on the wrong patient or wrong site of the patient's body. The basic components of the timeout are: positive identification identifica-tion of the patient; preoperative marking of the site of surgery; reviewing the patient's records and x-rays to confirm the type and location of the surgery; assuring as-suring the patient Is properly positioned for surgery; making sure any special equipment or device needed for the procedure Is available; and checking to make sure any required antibiotics antibi-otics have been administered An important part of this process is to actively involve the patient and family in the checklist's check-list's completion. It is expected that patients will be awake and take part in the identification and marking process. When you're a patient at the hospital, it is appropriate for you to ask your doctor about this process. In the operating room, the final fi-nal timeout is done just before beginning the operation, after you're asleep. A checklist is done of each of the components with eye contact among everyone ev-eryone in the room, including surgeons, assistants, nurses, scrub technicians and anesthesia anesthe-sia personnel. Our local hospitals use these basic safety initiatives to protect you and your family members from preventable harm. We encourage you to be engaged in this process and take personal responsibility to easure that this is being done for you and your loved ones.
