Show Fatally Stricken V DEATH CLAIMS S. t, PHYSICIAN Funeral services for Dr. Joseph Erie Jack. 46, a member of the surgical staff of L. D. 8. hospital and physician la Salt Lake City for many years, were being arranged ar-ranged Wednesday, following his death Tuesday night i Dr. Jack was stricken at his home, 1420 Gilmer drive, at t:45 p. m shortly after returning from a meeting la the Yale L. D. & ward chapeL The Salt Lake City' ' fir department rescue squad administered ad-ministered oxygen, but Dr. Jack was pronounced dead at 10:15 p. m. Dr. Jack was born In Salt Lake City October 31, 1893, a son of the late William H. and Annie Meeks Jack. He was graduated In 1918 from the University of Utah and In 1920 from the College of Physicians Phy-sicians and Surgeons of Columbia university, New York City. He served his Interneshlp at the Staten Island hospital.. New York City. He married Edith Forsberg, September Sep-tember 10, 1919, In the L. D. S. temple. Salt Lake City. He was a member of the Salt Lake County Medical society, the Utah Slate Medical society and the American Medical association, and was a fellow of the American Col-leg Col-leg of Surgeons.
