Show SUMMONS AND NOTICE TO PARENTS AND KINDRED OF SAID CHILD Case No. 29340 In the Juvenile Court of the Second District in and for Salt Lake County, State of Utah, Before Honorable Regnal Reg-nal W. Garff, Jr., Judge STATE OF UTAH In the interest of JOHN DOE, also known as BABY BOY BALLARD, an alleged al-leged neglected, abandoned and deserted child. TO: Leota Ponzo Ballard, Fort Hall Indian Reservation, Fort Hall, Idaho; Wilford Ballard. Fort Hall Indian Reservation, Fort Hall, Idaho; and, the kindred kin-dred of said child. WHEREAS, a petition having v been filed in the above entitled court on the 11th day of August, 1960, in the interest of the above named child alleging said child to be a neglected, abandoned and deserted child by reason of the fact that the mother and father of said child have abandoned and deserted said child and have willfully will-fully failed and neglected to provide pro-vide said child with subsistance, parental and other care necessary neces-sary to said child's health, morals and well-being: NOW, THEREFORE, you, the above named parents and the kindred of said child are hereby summoned and notified as follows: fol-lows: 1. To appear personally before the above entitled court in the said county on the 31st day of August, 1960, at the hour of 9:00 o'clock A.M. in the courtroom of said court located at 504 City and County Building, Salt Lake City, Utah, and show cause why said child should hot be dealt with according to law. 2. At said hearing your permanent perma-nent rights to the custody of said child will be adjudicated and the issue of whether or not said child should be placed for adoption adop-tion will be adjudicated. 3. You are advised that you are entitled to legal counsel if you so desire. 4. If you fail to appear, your default will be entered and the court will proceed to hear this cause and determine any rights or interest you may have in said child and will dispose of the case as may be determined according to law and the best interest of said child.
