needy is one of the fastest growing areas of state expenditures in Utah According to a study just completed by Utah I the private research organization medical assistance outlays multiplied about seven and half times between 1970 and 1984 These costs have continued to rise with the 1985 Utah Legislature appropriating nearly $195 million for this purpose in The state has made a number of efforts to control these medical assistance costs Two years ago the state began to reimburse hospitals for medicaid patients on the basis of a modified prospective payment system which makes the payment based on the average cost for each category of illness or injury rather than on the actual cost of a patient’s care This is similar to the system now used by the federal government for paying for medicare patients Thus far the system has had mixed reviews in Utah More recently a study by a management consulting firm recommended that Utah provide health care for most medicaid patients by contracting for advance care through alternate delivery systems such as health maintenance Under the (HMOs) organizations HMO concept a comprehensive set of health services is provided to enrollees at a fixed price which is contracted for in advance According to the Foundation report the the state began to implement recommendation this year by encouragin HMOs Between ing enrollment 1984 and May 1985 the December number of medicaid patients enrolled in an HMO plan rose by 27 percent At the present time individuals entitled to medical assistance may choose between the traditional system or one of the HMO plans Before a patient can be cniollcd under an HMO plan howcvci he oi she must sign a freedom of choice waiver Foundation analysts obseive that this requirement tends to limit pansion of HMO enrollments The state intends to promote the HMO concept by encouraging more medicaid patients to sign the freedom of choice waiver It also will seek a blanket freedom of choice waiver from the federal government If the state obtains this blanket federal waiver it eventually will require most medicaid patients living in one of the Wasatch ront counties to choose from one ol the HMO providers for then medical needs The Foundation report notes that medical assistance to the needy in Utah is provided under two main programs--thmedicaid or Title XIX program and the indigent medical assistance for low income persons not eligible under medicaid medicare or other health care programs Medicaid the largest of the two main programs is funded jointly with federal and state money The indigent medical assistance program is financed through state approperty tax propriations and a counties levy from participating care absorb Chronic and a maior traction ot medical assistance expenditures in
