Show When major catastrophic illness, financial crisis or an episodic tlareup of a chronic illness occurs to the senior population, it can be devastating. Unfortunately many independently functining elder-el- y are not prepared or adequately educated about the proper channels to access health care, financial support and living arrangments arrangements. Our county has an amazing variety of services available to the senior population. They often augment our facility and its services. Therefore a brief description of those programs and their relationship to our care center should be mentioned. A wide range of services are available which include bathing, general care, bandages, I.V.s and homemaker services, to mention a few. Longer term care may require alternative programs, Medicaid or self pay. Public health provides an invaluable service to those requiring specific needs in the home or outpatient services in the office. Referrals are also made by the physician or by personal contact by the patient. Specific tasks include blood pressure and diabetic monitoring. Both of these programs allow the senior patient to remain independent in their homes, or the home of a relative, for as long as possible. The Division of Family Services plays an active role in identifying those persons who need financial and medical assist-ancThey manage offices in both communities. Access to food stamps, physician care, hospital care and prescriptions are made e. available to qualifying individuals. Independent adult housing in a cloistered setting describes the adult living at the Delta Sands Apartments. These apartments are part of a federal program under the Farm Home Administration to provide low income housing to qualified recipients. these programs are resources to our own facility, All of the West Millard Care Center. The facility is centrally located in Delta across the street from the Delta Sands Apartments and the M.E. Bird Senior Citizen Center. We are a Intermediate Nursing Facility owned by Millard County and managed by Intermountain Health Care, Inc. We have been 36-be- an independently d standing the facility since 1985 when Delta Community Medical Center was completed at its present location. We have a close working relationship with the Delta Community and Fillmore Community Medical Centers, and are supervised by their administrator health, discharge services for the impaired, social dietary, pharmacy, emergency and planning, hearing services, Tn h' lab, xray, inpatient services, medical records and visiting physician services. We offer a wide range of activities to our residents while individually assessing their needs and abilities. We have an active recreational program under an excellent TRT and her volunteers. They have organized many enjoyable activities over the years. For two years in a row we entered a group exhibit in the Millard County Fair, and one year we received the Sweepstakes Award plus several individual first place awards. Other activities include our annual Mother's Day Program and Resident Christmas Program which are well Cosmetology students. and family participation. We have a group of dedicated and caring volunteers who donate many hours of service to our We could not residents. function without their help. Our visiting specialists provide a close and convenient service for our residents to avoid lengthy out of town trips. weekly for "van rides". The LDS and Church of Christ offer alternating Sunday services as well as a midweek Relief Society meeting. The Grace Baptist Church holds a weekly service. This past year the LDS church organizations have donated numerous gifts to the residents i in honor of their Relief Society's " sesquicentennial. Individuals and groups from the community have presented programs and offered their services for projects, and our facility has been designated as a participating facility for those as well. The children often host programs for the residents, and both children and residents share in special occasions such as Christmas and Halloween. The children enjoy the many loving "grandparents" in our facility and share a special relationship with them. Other resident services include a weekly visit from the School of Cosmetology at the Delta Vocational School. The residents enjoy a hair cut, curl or perm at the expert hands of the T.V. and a new cassetteCD player for our residents. Our Social Service Department hosts a family Support Group which meets monthly to discuss common problems and share matters of interest to the group. We are affiliated with the Alzheimers Association and invite family members from the community who are caregivers in the home to join us. We provide a State Certified Nurse Aide, Home Health and Nursery training Program which trains and qualifies nurses aides to complete the state examination and apply for jobs in these fields. Through IHC we offer assistance toward advanced nursing education for those employees who qualify.
