Sevier Valley Hospital Home Health Care can best be defined as skilled, part time services provided under physician's written order, in the residence of the homebound client. services may be a registered of those Skilled nurse, physical therapist, or therapist speech licensed nurse. A Home Health Aide may also be included to provide per- sonal care for the client. When the agency receives a referral from the physician a registered nurse makes an initial assessment visit to determine the patient's and care needs. eligibility A nurse, ther- apist andor an aide is then assigned and will call the client to set up a time for home visits. Most clients are seen for a six to eight week period until their health and level of independence is returned.
