Show J) mis Hospice Pr M&gif&s The Hospice program has now come to Davis Counly and citizens should be informed on this program prog-ram of Community Nursing Service Ser-vice throughout the county. HOSPICE of Davis County is now a program of Community Nursing Service, which is the oldest, old-est, continually operating home health agency in Utah. Community Nursing Service is a nongovernment, non-government, non-profit organization, organiza-tion, dedicated to providing competent, com-petent, compassionate home health care. It is a licensed, certified certi-fied home health agency, nationally national-ly accredited by the National League for Nursing and the Amer ican Public Health Association am1 is a member of the Utah Association Associa-tion of Home Health Agencies. The Hospice services include home care, which includes skilled nursing visits with 24 hours, on-call availability. Home health aide-homemaker aide-homemaker visits; volunteer visits; social worker and spiritual counselor available as needed and physical, speech and other supportive suppor-tive therapies. THERE ARE services to the be-reaved, be-reaved, follow-up services throughout the bereavement period. Family consultation serivce, information and referrals and support groups for bereaved. Hospice means ' Teamwork" the personnel work as a team, in cooperation with the patient's physician. Hospice nurses provide all nursing care along with supportive suppor-tive services from social workers, therapists, spiritual counselors, aides and dieticians. Family members mem-bers are also involved. Also available avail-able are the services of the Hospice medical director. CARE CAN be provided on a regularly planned basis or on an on-call basis, 24 hours a day, seven days a week. A vital part of Hospice is a net- ' work of trained volunteers who can assist in a wide variety of activities, such as petient care, funeral plan ning, supportive listening and crisis cri-sis intervention. THE ELIGIBILITY requirements require-ments are also given. Residents of Davis County are eligible regardless regard-less of age, sex, race, religion, national na-tional origin or financial status. t Hospice patients must have the approval and participation of their attending physician. Life expectancy expec-tancy must be defined in days, weeks or months.
