Here is how to enroll: •Pick up the required forms from the Family Child Care Office, Child Development Center, Youth Center or Family Support Center • Fill out AF Form 1181, Air Force Youth Flight Program Patron Registration •Fill out AF Form 356, Family Care Certification •Fill out Parent Memorandum of Agreement and Understanding of Air Force Extended Duty Child Care Program • Obtain written verification from your supervisor that extended care is needed •Provide the Family Child Care Office with a copy of the child's shot records •Once all of the paperwork has been turned in to the Family Child Care Office, specific reservations for the days and times care is needed can be scheduled through the Family Child Care Office with the provider during the week •Short notice requirements, during the weekend, can be scheduled with the Family Child Care providers •Parents may visit the home of the Family Child Care provider that is caring for their child as long as the child has been previously enrolled into the program trousers and bring gloves.
