The age at which children attain urinary control varies; therefore, it is difficult to specify a time at which an individual childs wetting is abnormal. known It should be remembered that ap- proximately 15 percent of otherwise completely normal children may still wet at age 5; and about 5 percent at age 10. In view of this, most parents need not be concerned about a bedwetting problem at all before age 5 unless there are signs of associated other urinary or general health problems. Generally speaking most children acquire the physical ability, and usually the desire, to control their own bladder function by the age of three years. The actual cause of bedwetting has yet to be clearly identified, although there are several theories, consequently there are varied approaches to treatment. It is important for the parent to try and identify a bedwetter that may be, at risk of having some more serious urinary disease. Basically, this would be any child who has never been dry at night and who has had some other urinary symptoms, such as persistent daytime wetting or other difficulties in emptying the bladder, or especially a history of urinary infection. Even many of these children will not be found to have a serious urinary disease. But they should be throughly checked. Children who were completely toilet trained, but at some later date started wetting, are referred to as secondary bedwetters. For these children the wetting is often a direct result of a traumatic event. If this event is significant enough, such a loss of a parent due to death or divorce, psychotherapy may be the answer to this childs problems. Children who come from families where there are or have been other bedwetters; who come from broken homes; or who are middle birth children, are more likely to be bedwetters. Occasionally, questions arise about the childs allergies or deep sleep as related to bedwetting. Food allergies as a cause of bladder irritation leading to bedwetting are extremely rare and consequently of low priority. Many bedwetters are also deep sleepers, but the cause and effect is poorly understood. What approach should a family take to a child who has a primary (never been dry) bedwetter, without any other problems? Basic good bladder habits are important. A child should not be allowed for instance, to empty its bladder only twice a day, morning and night, but should go to the bathroom at regular intervals. One good training method is positive reinforcement. In this approach the child is motivated to assume responsibility for its own learning, by keeping a progress chart or calendar. With steady regular improvement the child may be periodically rewarded. Punishment should never ever be used. A second method is the use of an electronic device with a bell, which wakes the child. These have been associated with some problems, such as skin irritation, but if a reliable device is used as directed, the results may be quite rewarding. The use of drugs can be quite successful but should be restricted to use under medical supervision only. A key word in dealing with a bedwetter is patience. The results of any treatment program are often a slow but steady improvement. At times one approach may fail but success results with another. If the bedwetting is associated with any other symptoms, or if there is any doubt, medical advice should be sought.
