The body will normally try to seal this rough spot by clumping platelets around it. With this clumping of platelets a clot can form, which may further obstruct the flow of blood or break loose and go downstream until it completely obstructs the .already narrowed vessel. When the blood supply in a coronary artery is totally obstructed, the heart muscle it supplies cannot receive any oxygen or other nutrients, and those muscle cells actually die. This heart muscle damage and cell death is actually what happens when a person suffers a heart attack. The process of fat deposition and narrowing of the arterial passageway is a Perhaps before going into these risk factors which are related to lifestyle and are things which each of us can personally do something about, we should discuss a little about what coronary artery disease is. Coronary arteries are arteries which carry blood to the heart muscle itself, providing the necessary oxygen and nutrients to those muscle cells to allow them to function and keep the pump, or heart, going. These arteries are so called medium-size- d arteries and seem to be particularly prone to a process of deposition within the wall of the artery, between its outer muscular layer and the inner lining of the artery. This fatty deposition process has been given the name atherosclerosis. As the wall of the artery becomes thicker, less fat-li- One of them which we cant do much about is heredity, if coronary artery disease exists in the familv. the likelihood that it will also exist in you as an individual patient is increased. It started me to think, how can I as a physician and patient educator help make people more aware of factors which contribute to coronary artery disease, the number one killer in America today. So, here goes. I have been playing around with words and letters to come up with a catchy word to help you remember what the five risk factors for coronary artery disease are. I wanted to make the word catchy, and yet one which would leave you with a feeling of dislike, so that even the hearing or mentioning of the word would make you loathe it and what it stands for. The word is SCABI. Now is that itchy and irritating enough? Heres what the letters of this acronyqi stand for .' blood can pass through is. Additionally, ' S: Smoking ke less significant. The signs and symptoms of coronary artery disease are quite well known because they have been talked about a lot in the media. They can be quite clear-cin their presentation or sometimes quite confusing. One significant fact should be emphasized; the presenting sign of coronary artery disease in 40 to 50 percent of heart attacks is sudden death. Thats pretty scary, and is one reason why we physicians emphasize prevention so much. Coronary artery disease progressing to the point of heart attack can be prevented in most cases if close ut gradual one, occuring over years and perhaps beginning even in childhood. Risk factors which contribute or speed up the process are quite well known. The most common symptom of coronary artery disease is chest pain, usually of a heavy or squeezing nature. It often follows exercise, emotional stress, or eating. It is usually located beneath the breast bone, and not out towards the left side of the chest, and may radiate up into the neck, jaw, shoulder, or inside of the left arm. Sometimes pain is not the dominant symptom, but tiredness or shortness of breath with exercise or activity. These symptoms may be warning signs that the heart muscle is not getting enough oxygen. Doctors think that most people have some warning signs before they have a major catastrophe. Obviously it is hard to interview those who have died suddenly of a heart attack, biit many times family members will indicate there have probably been some warning signs precede the sudden event. two years were elected at the an- e, -- 544-989- 4, JEFFERY bMJ L.
