LDS Hospital surgical oncologist R Dirk Noyes MD principal investigator for the Intermountain Region says that if the new procedure proves as effective as the current restandard moval of all lymph nodes in the region — the sentinel lymph node biopsy procedure could become one of the most important advances in breast cancer surgery in the lost several decades In the year since the clinical trial began over 800 women from around the country have enrolled in the study Utah places third in the nation in terms of patients enrolled with almost of the national enrollment Nationally researchers are hoping to enroll more than 5000 women for the study — the most comprehensive trial of its kind Dr Noyes is one of the national core trainers for the trial He has trained 40 surgeons including 21 in Utah — from Logan to StGeorge — to do the sentinel lymph node surin breast cancer gery patients As the leader of the trial in the West he reports thatmore surgeons are needed so that every woman in the state with breast cancer has this new staging technology available to her The procedure called sentinel lymph node biopsy involves the injection of a radioactive isotope and blue dye into the breast followed by removal of a few select lymph nodes from under the arm These "sentinel" lymph nodes are the first ones to which a orientation set STOP December 14 2000 at 10 a m in the Workforce to nearby lymph nodes Preliminary research indicates sentinel that if these few lymph nodes are free of cancer the disease has not spread and additional lymph nodes don't need to be removed If a sentinel lymph node is found to be cancerous however that indicates the tumor has spread to surrounding lymph nodes and more of the lymph nodes need to be removed "The problem is that no one knows what the impact of removing only the sentinel node is on the course of the disease" says Dr Noyes "We don’t know how it aftime or cancer recur- rence That's why this is an important study If women and their physicians have that information they can make better treatment decisions "This new procedure holds tremendous promise for women" says Dr Noyes "If we don't have to remove all the lymph nodes that drain the breast through more radical surgery we'd prefer not to This procedure may allow us to make a Highly effective diagnosis without subjecting patients to a far more invasive and painful surgery which also has more associated complications" All patients who enroll in the study will receive conventional surfor breast cancer — a gery lumpectomy or mastectomy to remove the breast tumor Patients will then be divided into two groups Women in the first group will have a sentinel node biopsy and if the sentinel node is positive will receive standard axillary dissection In the second group women who will receive no are further removal of lymph nodes All patients enrolled in the trial will receive appropriate radiation andor chemotherapy and hormonal therapy after surgery as needed according to the stage of each patient's cancer Patients will be followed closely by researchers for many years in order to record follow-udata on survival recurrence rates and complications To enroll
