The Millard School District Board of Education requires all students entering school for the first time to show evidence of a physical examination prior to admittance to district schools This g appl ie s in particular to enrol! in g and Kindergarten age children Evidence of such examination must be submitted to the respective school in which your child(rcn) is(arc) enrolled during the time of registration in the fall Physical examination forms can be obtained from any district elementary school or from the District Offices in : preregistration Middle for Deli Prcrcgistration School will be held on Wednesday August 18 1993 from 9 00 am to 1200 and p m Parents of returning 6th 7th 8th gTadc students may confirm students’ homeroom assignments and pay for lunch pledges in the DMS foyer Lunch applications should be completed and deposited at the Millard School District Office at 140 West Main in Delta before lunch payments are made at districts' schools Prcrcgistration for students new to the Delta area will be held in the DMS T students office on the same date and time Lunch applications need to be completed at the District office and paid for at DMS Parents of new students should have immunization records and grade reports from the last school attended for preregistration First day of school will be Wednesday August 25 1993 and it will be held from 8:20 a m to 3:05 pm Class schedules for 7th and 8th grade students will be distributed by homeroom teachers during period one on the first day of school Delta All physicians practicing in the Millard County area also have examination forms in their offices Students will NOT be admitted to school in the fall IF they have not completed the physical examination They will be excluded from school until the exam is completed If you have any questions in regard to the physical examination requirements please feel free to contact your local school or the school district offices Thank you Registration by mail for FES Fillmore Elementary School will once again do registration by mail for all students who were registered at the school as of last spring Parents should receive a registration packet in the mail sometime next week They are encouraged to go over it carefully and return to the school as soon as possible If any parents do not receive a registration packet by next week they should contact or they may pick the school at up a packet in person between the hours of 9 00 am and 12:00 noon or between 1:00 and 3:30 pm Parents of students who will be tending at Fillmore Elementary for the first time should come to the school and register their children in person New Red Hot!
