MISS FORILLO was born Oct. 4, 1960 in Ogden, a daughter of Frank V. and Dollie Collins Forillo. She was a certified nurses aid and had worked at Mt. Ogden Convalescent Center. She had lived in Kaysville up until a month ago when she moved to Riverside. She attended the Kaysville public schools and graduated from Davis High School. SHE WAS a member of the St. Rose of Lima Catholic Church in Layton. Surviving are her father of Kaysville, three brothers and one sister, Paul Forillo, Navato, Calif.; Charles L. Forillo, Robert F. Fori-l- o and Margie F. Wilson, all of Kaysville. She was preceded in death by her mother.
