The Columbia investigators are convinced that by correlating the rate of salivary flow with concentrations of sodium and potassium, they can narrow the diagnostic possibilities. When the ratio of sodium to potassium is below one to four, primary aldosteronism is most likely. When the proportion of sodium is somewhat higher, the disease is a strong possibility. But when the amount of sodium overbalances potassium, primary aldosteronism is ruled out. At present, diagnosis requires tests for urinary excretion of aldosterone which are expensive, time consuming, and not generally available. Therefore, quick, inexpensive salivary tests should simplify diagnosis in many cases. Furthermore, usually the ratio of sodium to potassium in saliva returns to normal three to five days after successful removal of adrenal tumors, whereas patients who do not benefit from such operations continue to have abnormal saliva. However, other forms of aldosteronism with confusingly similar symptoms are associated with severe kidney, liver, or y. heart diseases.
